UNACCOMPANIED MINOR FORM

Beginning at the January 18, 2024 meeting, PFLAG Peachtree City will now require a parent or legal guardian's phone number and signature for unaccompanied minors. We love hanging out with your students and we want to make sure they are safe!

We will keep these forms on file, so this will only need to be done once.

We will have printed copies at meetings. If you'd like, you can print the form that is below and bring it with you.
